Immigration Physical Checklist |

Before visiting PIC:
Ο Complete Part 1 of Form I-693 and I-693 supplement (if necessary)
Ο Obtain all medical records that might include information about vaccinations, hospitalizations, medications and other treatments. These records may come from such sources as the police, military, educational institutions or prior employers.
At time of initial visit please bring:
Ο To begin the physical just walk in to the Suwanee Premier Immediate Care anytime that this office is open.
O If Dr. Henderson is on duty you can have your examination and blood work/TB skin test done on this day. If Dr. Henderson is not seeing patients on your first day the physical can be performed on the day you need to return to have your TB skin test read or a day soon after this.
Ο Payment for physical ($250 for adults / $200 for children) is due at the start of this visit.
o We accept Visa, MasterCard, Discover, AmEx, check or cash
Ο Proper identification. All identification must be state or federally issued. At least one form must be a photo id. Acceptable forms of identification include:
o Passport
o Driver’s license
o Green card
o Birth certificate
o State issued identification card
o Military id card
Ο Alien Registration Number (if already assigned)
Ο All necessary paperwork including:
o Form I-693 & I-693 supplement
o Medical records (translated to English, if necessary)
o Immunization records (translated to English, if necessary)
In 48-72 hours:
Ο Return to the Suwanee office to have your TB test read. If you wait more than 72 hours past when your TB skin test was applied you may be required to have another skin test.
Ο If your physical was not already performed it may be done on this day.
Vaccination visit:
Ο A few days to a week after your TB skin test is read you will be asked to return for any required vaccinations to complete your I-693 immigration physical. Payment is due before vaccinations are administered.Ο Receive the original copy of your I-693 in a sealed envelope to give to the USCIS. You will be provided a photocopy of the document for your records.
